ABSTRACT

ABSTRACT Objective: To investigate whether the presence of obstructive sleep apnea syndrome (OSAS) alters the perception of respiratory symptoms and quality of life in COPD patients, by using specific questionnaires, as well as to determine whether scales for assessing daytime sleepiness and for screening for OSAS can be used in the triad of OSAS, COPD, and obesity. Methods: We included 66 patients diagnosed with mild-to-moderate or severe COPD and presenting with a body mass index > 27 kg/m2. After polysomnography, patients completed the Epworth sleepiness scale (ESS), the Berlin questionnaire (BQ), the modified Medical Research Council (mMRC) scale, the Baseline Dyspnea Index (BDI), and the Saint George's Respiratory Questionnaire (SGRQ). Results: Patients were first divided into two groups: COPD + OSAS (n = 46); and COPD-only (n = 20). The COPD + OSAS group was subdivided into a COPD + mild-to-moderate OSAS group (n = 32) and a COPD + severe OSAS group (n = 14), all of which were compared with the COPD-only group. There was a significant difference in mean FEV1 (L) between the COPD + OSAS groups and the COPD-only group (p = 0.073). The presence of the triad did not lead to significantly higher ESS scores, and scores > 10 had a specificity of 0.58. The BQ did not identify high risk for OSAS in the presence of the triad (specificity of 0.31). There were no significant differences in domain or total scores of the SGRQ between the COPD + OSAS groups and the COPD-only group. Conclusions: The confounding factors present in the triad of OSAS, COPD, and obesity prevented the perception of increased daytime sleepiness and high risk for OSAS. We observed no worsening of dyspnea perception or quality of life.


RESUMO Objetivo: Avaliar se a presença de síndrome da apneia obstrutiva do sono (SAOS) modifica a percepção de queixas respiratórias e de qualidade de vida em pacientes com DPOC por meio de questionários específicos, além de verificar se escalas de sonolência diurna e de rastreamento para SAOS podem ser empregadas na tríade SAOS, DPOC e obesidade. Métodos: Foram incluídos no estudo 66 portadores diagnosticados com DPOC leve/moderada ou grave e com índice de massa corpórea > 27 kg/m2. Após a polissonografia, foram aplicados escala de sonolência de Epworth (ESE), Questionário de Berlim (QB), escala modificada do Medical Research Council (mMRC), Baseline Dyspnea Index (BDI) e Saint George's Respiratory Questionnaire (SGRQ). Resultados: Foram analisados os grupos DPOC e SAOS (n = 46) vs. DPOC sem SAOS (n = 20). Do primeiro grupo, foram formados os subgrupos DPOC+SAOS leve/moderada (n = 32) e DPOC+SAOS grave (n = 14), que foram comparados com o grupo DPOC sem SAOS. Houve diferença significativa nas médias de VEF1 (l) entre os grupos DPOC com e sem SAOS (p = 0,073). A presença da tríade não aumentou significativamente o escore de ESE, tendo o escore > 10 especificidade de 0,58. O QB não identificou alto risco para SAOS na presença da tríade (especificidade de 0,31). Não houve diferenças significativas nos domínios e no escore total do SGRQ entre os grupos DPOC com e sem SAOS. Conclusões: Os fatores de confusão presentes na tríade SAOS, DPOC e obesidade impediram a percepção de maior sonolência diurna e de risco elevado de SAOS. Não foi identificada piora na percepção de dispneia e na qualidade de vida.

Abstract Introduction: Cardiorespiratory and musculoskeletal dysfunctions are common in the postoperative period of kidney transplant patients and are often accompanied by low exercise tolerance. Objective: The purpose of this study was to evaluate the impact of an early physiotherapy program during hospital stay on functional capacity and peripheral and respiratory muscle strength after kidney transplant. Methods: An open, randomized clinical trial was conducted in patients undergoing living donor kidney transplant. Sixty-three patients were included (intervention group-IG: n = 30; control group-CG: n = 33). IG received an early physiotherapy program from first postoperative day until hospital discharge and CG received standard care. The variables of interest were measured preoperatively and at discharge except for respiratory muscle strength and vital capacity (VC), which were also measured on the first postoperative day. Functional capacity was evaluated through six-minute walk test (6MWT); peripheral and respiratory muscle strength using a dynamometer and manovacuometer, respectively; and VC through spirometer. Results: After surgery, there was a reduction in functional walking capacity and peripheral muscle strength without different between groups (p > 0.05); however, respiratory muscle strength was significantly higher in IG (p < 0.001) at hospital discharge, when comparing with CG. Conclusions: An early physiotherapy program during hospitalization for patients undergoing living donor kidney transplant caused a lower reduction in respiratory muscle strength and without additional benefits in the functional capacity, when compared to a control group, although the clinical relevance of this finding is uncertain.


Resumo Introdução: Distúrbios cardiorrespiratórios e musculoesqueléticos são comuns no período pós-operatório de pacientes de transplante renal, e são frequentemente acompanhados por baixa tolerância a exercícios. Objetivo: O presente estudo pretendeu avaliar o impacto de um programa precoce de fisioterapia durante a internação sobre a capacidade funcional e força muscular periférica e respiratória após transplante renal. Métodos: Foi realizado um estudo clínico randomizado aberto com pacientes submetidos a transplantes renais com doadores vivos. Sessenta e três pacientes foram incluídos (grupo de intervenção GI: n = 30; grupo de controle - GC: n = 33). O GI recebeu o programa precoce de fisioterapia a partir do primeiro dia de pós-operatório até a alta hospitalar e o GC recebeu tratamento padrão. As variáveis de interesse foram medidas no pré-operatório e na alta, exceto por força muscular respiratória e capacidade vital (CV), que foram medidas no primeiro dia de pós-operatório. A capacidade funcional foi avaliada através do teste da caminhada dos seis minutos (TC6); força muscular periférica e respiratória com o uso de um dinamômetro e um manovacuômetro, respectivamente; e a CV por meio de um espirômetro. Resultados: Após a cirurgia houve reduções na capacidade funcional de caminhar e na força muscular respiratória sem diferenças entre os grupos (p > 0,05); contudo, a força muscular respiratória foi significativamente mais elevada no GI (p < 0,001) no momento da alta hospitalar em comparação ao GC. Conclusões: O programa precoce de fisioterapia oferecido durante a internação dos pacientes submetidos a transplantes renais com doadores vivos produziu uma menor redução da força muscular respiratória e não resultou em benefícios adicionais na capacidade funcional, apesar da relevância clínica desse achado ser incerta.

ABSTRACT

ABSTRACT The treatment of COPD has become increasingly effective. Measures that range from behavioral changes, reduction in exposure to risk factors, education about the disease and its course, rehabilitation, oxygen therapy, management of comorbidities, and surgical and pharmacological treatments to end-of-life care allow health professionals to provide a personalized and effective therapy. The pharmacological treatment of COPD is one of the cornerstones of COPD management, and there have been many advances in this area in recent years. Given the greater availability of drugs and therapeutic combinations, it has become increasingly challenging to know the indications for, limitations of, and potential risks and benefits of each treatment modality. In order to critically evaluate recent evidence and systematize the major questions regarding the pharmacological treatment of COPD, 24 specialists from all over Brazil gathered to develop the present recommendations. A visual guide was developed for the classification and treatment of COPD, both of which were adapted to fit the situation in Brazil. Ten questions were selected on the basis of their relevance in clinical practice. They address the classification, definitions, treatment, and evidence available for each drug or drug combination. Each question was answered by two specialists, and then the answers were consolidated in two phases: review and consensus by all participants. The questions answered are practical questions and help select from among the many options the best treatment for each patient and his/her peculiarities.


RESUMO O tratamento da DPOC vem se tornando cada vez mais eficaz. Medidas que envolvem desde mudanças comportamentais, redução de exposições a fatores de risco, educação sobre a doença e seu curso, reabilitação, oxigenoterapia, manejo de comorbidades, tratamentos cirúrgicos e farmacológicos até os cuidados de fim de vida permitem ao profissional oferecer uma terapêutica personalizada e efetiva. O tratamento farmacológico da DPOC constitui um dos principais pilares desse manejo, e muitos avanços têm sido atingidos na área nos últimos anos. Com a maior disponibilidade de medicações e combinações terapêuticas fica cada vez mais desafiador conhecer as indicações, limitações, potenciais riscos e benefícios de cada tratamento. Com o intuito de avaliar criticamente a evidência recente e sistematizar as principais dúvidas referentes ao tratamento farmacológico da DPOC, foram reunidos 24 especialistas de todo o Brasil para elaborar a presente recomendação. Foi elaborado um guia visual para a classificação e tratamento adaptados à nossa realidade. Dez perguntas foram selecionadas pela relevância na prática clínica. Abordam a classificação, definições, tratamento e evidências disponíveis para cada medicação ou combinação. Cada pergunta foi respondida por dois especialistas e depois consolidadas em duas fases: revisão e consenso entre todos os participantes. As questões respondidas são dúvidas práticas e ajudam a selecionar qual o melhor tratamento, entre as muitas opções, para cada paciente com suas particularidades.

OBJECTIVE: The effects of acute continuous positive airway pressure therapy on left ventricular diastolic function and functional capacity in patients with compensated systolic heart failure remain unclear. METHODS: This randomized, double-blind, placebo-controlled clinical trial included 43 patients with heart failure and a left ventricular ejection fraction <0.50 who were in functional classes I-III according to the New York Heart Association criteria. Twenty-three patients were assigned to continuous positive airway pressure therapy (10 cmH2O), while 20 patients received placebo with null pressure for 30 minutes. All patients underwent a 6-minute walk test (6MWT) and Doppler echocardiography before and immediately after intervention. Clinicaltrials.gov: NCT01088854. RESULTS: The groups had similar clinical and echocardiographic baseline variables. Variation in the diastolic function index (e′) after intervention was associated with differences in the distance walked in both groups. However, in the continuous positive airway pressure group, this difference was greater (continuous positive airway pressure group: Δ6MWT = 9.44+16.05×Δe′, p = 0.002; sham group: Δ6MWT = 7.49+5.38×Δe′; p = 0.015). There was a statistically significant interaction between e′ index variation and continuous positive airway pressure for the improvement of functional capacity (p = 0.020). CONCLUSIONS: Continuous positive airway pressure does not acurately change the echocardiographic indexes of left ventricle systolic or diastolic function in patients with compensated systolic heart failure. However, 30-minute continuous positive airway pressure therapy appears to have an effect on left ventricular diastolic function by increasing functional capacity. .
